2. Medical Release Form
3. Proof of Residency – 3 Items
   - See Residence Eligibility Requirements
   - See League Boundaries
4. Volunteer Form
5. Checkbook or Cash – Checkout Form
6. Birth Certificate – New Players Only
   - Original birth certificate only!
   - NO COPIES, No Passports, No Hospital Records
7. Coaches & Managers
   1. Copy of Driver’s License
   2. Completed Background Check Form

Fees:

• $100 T-Ball (Ages 5-6)
• $125 Farm (Ages 7-8)
• $125 Minors (Ages 9-10)
• $125 Majors (Ages 11-12)
• $150 Juniors (Ages 13-14)
• $150 Seniors (Ages 15-16)

Fees Include: Shirts, Socks, Cap and Pancake Breakfast.

Late Sign-ups Add $25.00 (after January 29, 2010)
